ULTRA Accident Investigation
The primary purpose is to discover the principle cause of
the accident and report the accident. The site supervisor or area supervisor will
investigate all accidents as soon as possible after they
occur. The supervisor or person conducting the accident
will take an impersonal fact finding attitude.
The accident reporting form (see attachment) should be used
to document and report any accident to the company safety
director. The purpose
of the accident investigation is corrective, and not a disciplinary
action.
In addition to the above requirements, any additional requirements
by our customer’s safety procedures must and will be followed.
The following guidelines are established for reporting
hazardous conditions or an accident on site:
Immediately contact the site or area supervisor to report
the hazard
or accident
The area or site supervisor will contact the company (customer)
person on call or emergency person on call and report the
accident
or hazard
Follow the company’s established guidelines for dealing
with an
emergency and following up with an accident report
Take all necessary steps to prevent further injury and manage
the
situation
